Double posts

All over the boards people double post left, right and centre.
It's never happened to me. Just out of curiosity (so I know what to avoid doing) how does it happen?

Generally it happens when people think too much as repeatedly hit the Submit Button because the server has slowed down a little.

People also hit submit then the stop button on their browser to edit their posts before resubmitting-
